Health Update - June 2023

Hello everyone. In this video you will get a small update on Dianna's PICC line. It is used to give her medications and fluids for hydration without her having an adverse reaction to it.

OVERALL HEALTH - Dianna is still daily bed ridden, barely able to move. For the past month or so, she has had her ups and downs. Dianna continues to have short windows of time where she can talk for a couple minutes, or maybe play a short tune on her tiny piano. These short positive times are usually followed by a crash for the remaining few days to a week.

For anyone a little new to her situation - She has her daily long chronic fatigue, as well as her MCAS which is causing her to have allergies to essentially and everything you can think of. Artificial smells cause her brain fog, and a long list of physical issues. Also 99% of all foods cause her body to react in poor ways.

(Direct from Levi, her editor and videographer) This month I've had started helping Kyle care-take for Dianna full time. I must say, though I know her situation, it is a completely different experience being here all day. She genuinely has been in a dark room, for many months, with no stimulation, and nothing to do but "try to recover". She does not watch TV, or read, and she can barely sleep. She's awake all day in a room while Kyle, me, and others help her get her food, medications, clean laundry, etc. (She cannot have stimulation of any kind or else her body crashes really hard). I wish I could articulate how surreal this whole thing is. But it is hard to describe what it is like actually being here and seeing someone not only in such poor health, but with no fun outlets and hardly and end in sight.

Dianna's family and friends visit when they can, and there is a lot of hope in the air. We get little glimpses of the old Dianna when she is feeling better and making jokes. It really brightens up everyone's week, even if they are only for tiny periods during the month. We all plan to be at her side while she see's this thing through. None of this has killed her silly sense of humor. We'll have Dianna back some day, hopefully explaining to us the science about this whole thing.

Here’s what I found on the CDC’s page on ME/CFS symptoms (I’d post a link but I’m not sure what all is appropriate for posting on patreon yet since I’m new around these parts!): Worsening of ME/CFS symptoms after physical or mental activity that would not have caused a problem before illness. This is known as post-exertional malaise (PEM). People with ME/CFS often describe this experience as a “crash,” “relapse,” or “collapse.” During PEM, any ME/CFS symptoms may get worse or first appear, including difficulty thinking, problems sleeping, sore throat, headaches, feeling dizzy, or severe tiredness. It may take days, weeks, or longer to recover from a crash. Sometimes patients may be house-bound or even completely bed-bound during crashes. People with ME/CFS may not be able to predict what will cause a crash or how long it will last.

The only small thing with a little but scientifically significant results so far seems to be with Coenzyme Q10 or CoQ10 and about our cells' energy producers call mitochondria. Hope your/Dianna's team is considering everything. Here're a couple of studies. I"m glad that you found (presumably) an adhesive to hold the PICC line in place that doesn't elicit reactions from Dianna's immune system. I've had PICC lines a few times, and will again soon (for cancer treatment), and tend to develop allergy to those adhesives myself. But I'm relieved to know that Dianna doesn't have to be poked as often with needles now. When I get a PICC line, I amuse myself by saying that I'm being "Borg-ified," as in the cybernetic oranisms in *Star Trek: The Next Generation.* Now I'm also in extra good company, with Dianna being similarly accoutred.

I just thought of a silly idea for the upcoming Open Sauce. What if Allen (watching his vid when I thought of it) or really a group of creators work on a Diana cam so she can attend virtually on the shoulder of her friends. Maybe we make it a parrot, or other puppet/animal/fruit whatever, with a cam and mic inside that people can interact with. So important that you are sharing these details. The daily life of chronic illness is so hidden that it hardly seems real to most people. I will do that. But try to remember my original advice was to not give up, try to find solutions, and fight. The phrase "force your body to handle more than it could before" is not about pretending like you're perfectly healthy. It's advice meant to drive for action. Responders have focused on the exercise bit, but none of you seem to value mental strength. I did also say "mentally engage". The essence of my advice was positive, but you didn't notice it. I think you, and many rational people forget that it is good to try, experiment, stay positive, because you are too comfortable waiting around for all the evidence (that someone else will collect for you...). I will watch your documentaries, and I'm sure will learn something, but it does not seem like you are much concerned with learning from anyone else. I think this ultimately, will likely be a weakness. I'm sorry you've struggled, and I hope you find your way out of what you're in. But no one will save you from your predicament. Your attitude must go along with the medicine. Drive yourself to health. Much love. ❤

Long Covid, MCAS, and MECFS are caused by viral infections and do not respond well to exertion. It will cause PEM/PESE that is disabling and can be permanent. With all due respect, your is horrible “advice” and your presumptive take that what works for most illnesses works for viral illnesses is what’s wrong with the medical establishment. I ignorantly “pushed through” LC for 2 years and drove myself to bedridden. Prayed it wouldn’t be permanent. A year later I am still disabled. A person can’t begin to rebuild exertion until they’ve found a cocktail of meds that will help prop up their body, and reduce systemic inflammation and immune system over-response. There’s no clear cut cocktail, so it takes a ton of trial and error. Likely due to viral persistence.

Perhaps you're right. I'm only being pragmatic, because it seems like this is taking a while and going in circles. I know from experience that especially with new diseases, doctors do not always know how to proceed, and it's often up to you to ad lib. It sounds unscientific, but it is simply early science: experimentation. Sometimes failure means you should give up a course of action, sometimes it means the improvement is too negligible to understand as improvement, and you must keep fighting, for a negligible improvement to become a substantial one. Waiting around may work for colds and short term viruses, but for a long term disease, to shorten recovery from say, two decades, to a few years, it may take a healthy amount of stubbornness. All I'm preaching, is to trust not only your doctors, but also dare to trust yourself, and experiment with your own condition, and try to see what works. You may be surprised (or unsurprised) at the results. Of course, everyone is free to choose what is best for them, but every one of our experiences is valuable to each other, including yours, and including mine. I was sharing my own experience. When I was recovering from CMV, which ended up causing Transverse Myelitis, forcing my body to do more each day was exactly what ended up causing the TM to get worse. Generally, yes, exercise is the path to the recovery, but not always. It depends on the person. These days, when I have a virus, I always sleep and rest until it is gone - however long that takes, and then move forward with recovery.

Briefly, concerning the issue: the virus seems to vampirically take your cells energy. But I'm assuming over time, your body does reduce the presence of the virus, perhaps it can grow again, but there are windows of opportunity. Wouldn't maintaining the fight, maintaining some level of exercise, and occasionally pushing to find your limits, keep everything running as efficiently as possible, so that when your immune system does respond well to the virus, it offers an opportunity to create a positive feedback loop? The only issue is balance. Find *your* max efficiency. But it doesn't happen on its own, it happens by *trying* things and not giving up. You don't need to be a doctor to understand this logic. It's always dangerous when the stakes are high... but what else can you *do* ? Much love, keep up the fight. ❤👊

It is mentioned for the average patient, that slowly increasing exercise (implied, depending on the patient) is still, ideally under supervision, the recommend path to recovery. I'm glad to hear some amount of progress. Also really interesting to see a PICC line where they use a needle in it. Here in New Zealand we have a self-sealing system that our Syringes screw onto that uses a positive pressure system to prevent bacteria or air getting in during the connection or disconnection of it. I echo everything Dr Andy wrote. I had a PICC line for transfusions, antibiotics, blood works and chemo for several months. It had to be flushed, and the area sterilized weekly when it was flushed. I'm not sure what Chris meant about the nurse changing the PICC line. Maybe he meant the line being flushed.

Being trapped in just your mind is such a hard experience. Though the mind is extremely resilient if you let it be. Working on problems without a pencil and paper is a different experience but it is possible. My chronic fatigue has put me in this state for weeks but not months. When in that state I use the couple minutes I have to take in a physics problem. Then think about it for days until I have 5 minutes I can record my thoughts. Today is today and all we can do is accept the reality of today. Nobody knows what the reality of tomorrow is, but focusing on making it better is a worthy struggle. We are all sending you positive thoughts.

Surreal, indescribable suffering is probably the best way to describe what long-haul appears to be to loved ones. To me, it was a profound realization of how immensely a person can suffer and still be alive. I hate that she's living through this, but I'm hoping she gets better faster than my wife did (~2yrs later, I have her back ~100%).
